How to request a copy of your Form 1095-C:

Requests must include your full name, the year requested, and a current mailing address or preferred email address for delivery.
Upon request, a copy of Form 1095-C will be provided within 30 days of the request or by January 31 of the following year, whichever is later.
Please note that you do not need Form 1095-C to file your federal income tax return, but it is available for your records if you choose to receive it.
